Collection of Application Forms (No quota)

1. Collect application form from school in person.

Office Hour：

Monday to Friday (8:00a.m. to 6:00p.m.)

Saturday (8:00a.m. to 1:00p.m.)

2. By Fax or E-mail.

3. Download the application form .

Submission of Application(No quota)

- Deadline: Applications can be submitted throughout the year

- Submission ：In person / By mail / Fax / E-mail

Documents to be submitted with the application form:

1. Application form

2. Copy of child's birth certificate

3. Copy of immunization record

4. 2 self-addressed stamped envelopes

5. Application fee HKD40, If submitting by cheque, please make the cheque payable to: Pok Oi Hospital Sy Siok Chun Kindergarten, and write the applicant's name on the back of the cheque (The application fee is non-refundable, whether the application is successful or not)
